*** IMPORTANT : The yearly club membership fee is divided into monthly payments payable in advance for a particular month. January will be billed Pro-Rata depending on opening date. February to November will be billed in full per month. December will not be billed. If you join late in the year your membership fee is calculated Pro-Rata for the particular month you joined and the remaining months billed in full per month. Each month your membership is active will be billed accordingly, if you wish to cancel your membership a one calendar month notice must be given by sending an email to info@midstream.club.

Junior FAP League Matches and Training – R1210 per month – 2025 Branded Kit (Compulsory) must be purchased separately at R1120 (Shirt, Shorts and Socks) or 2025 Branded Goal Keeper Kit at R1330 (Shirt, Shorts and Socks).

        GDPL League Matches and Training – R1400 per month – 2025 Branded Kit (Compulsory)          must be purchased separately at R1120 (Shirt, Shorts and Socks) or 2025 Branded Goal          Keeper Kit at R1330 (Shirt, Shorts and Socks).

Skills: All Ages

Private Coaching: All Ages 

Private Training Only – R455 per 1 hour session per person – 2025 Branded Kit (Optional) can be purchased separately at R1120 (Shirt, Shorts and Socks) or 2025 Branded Goal Keeper Kit at R1330 (Shirt, Shorts and Socks).

Filling in this Registration Application does not guarantee membership in the club. Once complete we will be in contact to confirm membership and team allocation.

Club Facility Rules & Safety Information

• The Club reserves the right of admission.
• Alcohol, illegal drugs, smoking, vaping and e-cigarettes are strictly prohibited anywhere on the club premises.
• Only coaches and registered players are permitted on the playing fields. Parents, guardians and spectators must remain on the embankments or designated viewing areas unless given express permission by management.
• Do not place, leave or discard any objects, litter, equipment or rubbish on the fields or surrounding areas – this creates hazards for others.
• Do not move, adjust or tamper with rocks, logs, markers, goals, nets or any other equipment without permission from a coach or management.
• If there is an injured player notify the nearest coach or management for assistance and to call for paramedic or ambulance services if needed.
• All players and participants must be medically fit to train or play. Do not participate if you are unwell, injured, or under the influence of alcohol, medication, drugs or any prohibited substance that may impair your ability.
• Children under 12 must be accompanied and supervised at all times by a parent or guardian. Parents/guardians are fully responsible for the behaviour and supervision of their children at all times.
• Respect all players, coaches, officials, parents and spectators. Bullying, harassment, discrimination, abusive language or unsportsmanlike behaviour will not be tolerated.
• No unauthorised coaching, training sessions or commercial activities on the fields without written management approval.
• Littering, vandalism or damage to club property will result in immediate removal and possible prosecution.
• No player is allowed to participate in any activity unless the player is a registered club member or part of an official approved visiting team.
• In case of bad weather, lightning or storms, leave the fields and premises immediately. Severe weather can cause serious injury or death.

Liability & Indemnity

•  Persons using these facilities, parking areas, fields and structures do so entirely at their own risk. The facility owners (Bondev Midrand (Pty) Ltd), the lessee (Club Football at Midstream (Pty) Ltd) and their employees, officers, directors, agents and any third parties accept no responsibility or liability for any theft, loss, damage, injury or death of whatsoever nature, howsoever caused, including (but not limited to) negligence. You hereby release, hold harmless and forever discharge Bondev Midrand (Pty) Ltd and Club Football at Midstream (Pty) Ltd, their employees, officers, directors, agents and any third parties from any claims, demands, actions or causes of action arising out of or related to any loss, property damage, personal injury (including death) sustained while on the property.

 Training and Match Specific Rules

•  Training sessions to run on time at the agreed times set. If a player is late for a training session, it can unfortunately not be extended beyond the specified time. Once a training session has begun no player may enter the field before being invited in by the Coach
• Players may only leave the area when dismissed by the Coach, should you require to leave early please notify the Coach in advance
• No eating during a lesson, refreshment breaks will be provided
• Training sessions cancelled by the Club for valid reasons will be made up where possible
• Coaches are there to coach the players, always respect the coaches and their decisions at training and matches. Follow our club communication protocol if you need to raise concerns.

 Club Membership and Billing Rules

•  All billing is done in advance for a particular month and payment due in advance
• All membership options require one full calendar month notice period for cancellations; the one calendar month notice period will still be billed and is payable
• Non-payment will result in your membership being cancelled
• No credit or reduction in fees will be granted for non‐attendance due to illness, injuries or for any other reason
• League clearance certificates will only be given if account is in good standing

General

• Club Members/Players/Parents/Guardians/Visitors consent to the Club (Club Football at Midstream (Pty) Ltd) and its commercial partners recording, retaining and reproducing image and voice by way of photographs, sound recordings and video pertaining to the Club (Club Football at Midstream (Pty) Ltd) activities in a group setting at our facilities.

Club Communication Channels and procedure to follow

Coaches are there to coach the players, always respect the coaches and their decisions at training and matches. Follow our club communication protocol if you need to raise concerns.

Team WhatsApp groups are there for communicating training and match related information. Good natured comments and team encouragement as well as photos or videos of match days can be shared. The WhatsApp group are NOT for complaining or negative comments. Follow our communication procedure to raise any relevant concerns.

1.     Your first point of communication regarding training, coaching or matches should be with your relevant team coach. If you need the number of the team coach then ask on your team WhatsApp group or check on the APP. Please approach your coaches with a calm and friendly demeanour.

2.     If your team coach can’t help or you wish to address an issue with management about a training, coaching or match related issues please contact Graham our Football Manager or make an appointment for face-to-face. (Email: go@midstream.club). Available at the club for face to face appointment on Tue & Thu from 16h30-18h30.
